29 CFR 1910.178(p)(1):Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s). a) On or about May 06, 2025, at the company's yard, employees were exposed to struck-by and crush hazards, in that, the Senstate Doosan Long Forklift (Serial No. FDAOW193002970) amber strobe light was loose and hanging from its wiring. b) On or about May 06, 2025, at the company's yard, employees were exposed to struck-by and crush hazards, in that, the Senstate Mitsubishi Forklift (Serial No. AF17EZ0843) amber strobe light was damaged. c) On or about May 06, 2025, at the company's yard, employees were exposed to struck-by, electrical and crush hazards, in that, the Moffett Lift the (Serial No. U050698B) amber strobe light electrical wires were exposed, was leaking oil, a hydraulic hose was broken and wheels were damaged. d) On or about May 06, 2025, at the company's yard, employees were exposed to struck-by and crushing hazards, in that, the Hyundai Forklift (Serial No. HHKHFV36CF0000711) had a broken windshield. Oldcastle APG South, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(p)(1), which was contained in OSHA inspection number 1703810, citation number 1, item number 1 and was affirmed as a final order on 2/5/2024, with respect to a workplace located at 4751 Hamilton Blvd, Theodore, AL, 36582.
