At the worksite, located near Hidden Hammock Loop Parrish, FL 34219; On or about 3/25/25, the employer exposed employees to struck by and crushing hazards in that an excavator and loose soil were left unsecured on the excavation edge while sewer line repair operations were being conducted within an excavation approximately 16ft in depth.
