29 CFR 1904.39(a)(2):The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: (a) On or before March 8, 2025, the employer failed to report the hospitalization of an employee after a serious injury which occurred at the hydrostatic testing site. The employee sustained injuries on March 8, 2025 at approximately 10:00 AM and OSHA was not notified of the hospitalization until March 10, 2025 at 12:00 PM.
