29 CFR 1910.119(m)(4)(iii):The employer's incident investigation report did not includes a description of the incident: Cargill, Inc. DBA Cargill Meat Solutions Corporation, 1505 East Burlington Avenue Fort Morgan, CO 80701: The employer failed to ensure that the incident investigation reports were descriptive,and contained details of the following incidents.: (a) On or about February 6, 2025, the incident investigation for an ammonia release involving pipe corrosion for piping lines CS 100 and 101 located on the penthouse roof did not contain a clear explanation/account of the incident. (b) On or about February 10, 2025, the incident investigation for an ammonia release involving a malfunctioning pressure relief valve (PRV) located on the sub-roof liquid header did not contain a clear explanation/account of the incident.
