29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment. a) On or about January 28, 2025, at the facilities located at 2430 East Glendale Avenue, Appleton, Wisconsin; the employer did not verify through written certification that a workplace hazard assessment for personal protective equipment (PPE) had been performed when employees working in the Manufacturing, Converting, and Maintenance areas were provided and required to use PPE such as but not limited to, eye protection, foot protection, ear protection, and other specialized PPE. b) On or about January 28, 2025, at the facilities located at 2441 East Glendale Avenue, Appleton, Wisconsin; the employer did not verify through written certification that a workplace hazard assessment for personal protective equipment (PPE) had been performed when employees working in the Manufacturing, Converting, and Maintenance areas were provided and required to use PPE such as but not limited to, eye protection, foot protection, ear protection, and other specialized PPE. c) On or about January 28, 2025, at the facilities located at 2239 East Glendale Avenue, Appleton, Wisconsin; the employer did not verify through written certification that a workplace hazard assessment for personal protective equipment (PPE) had been performed when employees working in the Manufacturing, Converting, and Maintenance areas were provided and required to use PPE such as but not limited to, eye protection, foot protection, ear protection, and other specialized PPE.
