29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent: On or about December 30, 2024, and at times prior thereto, the employer did not record work related employee injuries in the following instances (a-b): a) On or about November 13, 2024, employee two (2) suffered a work related injury will working at shredder # 3 that required stiches. b) On or about December 2, 2024, employee one (1) suffered a work related amputation while working at shredder # 3.
