29 CFR 1910.147(c)(5)(ii)(D):Lockout devices and tagout devices did not indicate the identity of the employee applying the device(s): a) Building B Line 3/Mixing Horizontal Mixers - On or about February 18, 2025, and at times prior, the employer exposed employees to electrical shock and struck by hazards in that, the employer failed to ensure that all lockout and tagout devices indicated the names and identity of the employees applying locks and tags while they were performing sanitation operations on the equipment.
