29 CFR 1904.39(a)(1): Basic Requirement. Within eight (8) hours after the death of any employee as a result of a work-related incident, you must report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor. a. On or about December 13, 2024, a work-related fatality of an employee occurred at the workplace. The employer did not report the fatality to OSHA within 8 hours.
