29 CFR 1904.29(b)(3): The employer did not enter each recordable injury or illness on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness had occurred: a.) Viking Tool & Gage, Inc. 11210 State Highway 18 Conneaut Lake, PA 16316: on or about 12-05-2024, and times prior thereto: an employee sustained a recordable injury while working in the Stamping Area. The injury was not recorded on the 300 log within 7 calendar days of the injury. As of 12-17-2024, the OSHA 300 Log was not completed for the injury. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
