29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident. On December 5, 2024, an employee was operating a tractor with an auger attachment to dig post holes when the employee got caught in the auger, resulting in fatal injuries. The fatality was not reported within 8 hours to the local Area Office of the Occupational Safety and Health Administration.
