29 CFR 1910.119(g)(3):Training documentation. The employer shall ascertain that each employee involved in operating a process has received and understood the training required by this paragraph. The employer shall prepare a record which contains the identity of the employee, the date of training, and the means used to verify that the employee understood the training: a)AMTEC Corporation dba Tech Ord, a Division of AMTEC Corporation, 47600 180th Street, Clear Lake, SD 57226: On or about and at times prior to November 25, 2024, the employer did not ensure employees operating the Sweco Vibratory Screener to process pyrotechnics materials had completed training on the Machine Operating Procedure Sweco Vibratory Screener Document Number 600086-050 Revision F by preparing a record which contained the identity of the employee, the date of training, and the means used to verify that the employee understood the training. This condition exposed employees to fire and explosion hazards. b)AMTEC Corporation dba Tech Ord, a Division of AMTEC Corporation, 47600 180th Street, Clear Lake, SD 57226: On or about and at times prior to November 25, 2024, the employer did not ensure employees preparing PY-951 had completed training on the Product Realization Operating Procedure Manufacture of PY-951 Document Number 710054 Revision N by preparing a record which contained the identity of the employee, the date of training, and the means used to verify that the employee understood the training. This condition exposed employees to fire and explosion hazards. c)AMTEC Corporation dba Tech Ord, a Division of AMTEC Corporation, 47600 180th Street, Clear Lake, SD 57226: On or about and at times prior to November 25, 2024, the employer did not ensure employees preparing I-308 had completed training on the Product Realization Operating Procedure Manufacture of I-308 Document Number 7000206 Revision L by preparing a record which contained the identity of the employee, the date of training, and the means used to verify that the employee understood the training. This condition exposed employees to fire and explosion hazards. d)AMTEC Corporation dba Tech Ord, a Division of AMTEC Corporation, 47600 180th Street, Clear Lake, SD 57226: On or about and at times prior to November 25, 2024, the employer did not ensure employees operating the Mikro Samplemill to process pyrotechnics materials had completed training on the Machine Operating Procedure Mikro Samplmill (Hammer Mill) Document Number 600092-050 Revision C by preparing a record which contained the identity of the employee, the date of training, and the means used to verify that the employee understood the training. This condition exposed employees to fire and explosion hazards. AMTEC Corporation dba Tech Ord, a Division of AMTEC Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.119(g)(3), which was contained in OSHA inspection number 1627407, citation number 1, item number 5b and was affirmed as a final order on September 20, 2023, with respect to a workplace located at 47600 180th Street, Clear Lake, SD 57226.
