29 CFR 1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6). a) Airstream, Inc. on or about November 25, 2024 at the location of 1001 West Pike Street Jackson Center, Ohio 45334: The employer did not ensure employees tasked with performing lock out covered tasks, followed established procedures in sequence to ensure equipment was fully deenergized. Due to this condition, employees tasked with die changes on the Minister Punch Press (TS030) were exposed to pinch point hazards. The Airstream, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.147(d), which was contained in OSHA inspection number 1705579, citation number1, item number1 and was affirmed as a final order on March 7, 2024, with respect to a workplace located at 1001 West Pike Street Jackson Center, Ohio 45334.
