29 CFR 1904.29(b)(3): Each recordable injury or illness was not entered on the OSHA 300 Log and an incident report (OSHA Form 301 or equivalent) within seven (7) calendar days of receiving information that a recordable injury or illness has occurred: a. On or about July 8, 2024, at the above located workplace, a recordable injury occurred in the processing room. The employee sustained a knife laceration to left hand, resulting in five days away from work. The employer did not record the injury on an OSHA 300 Form, OSHA 301 Incident Report, or their equivalent form for calendar year 2024. b. On or about October 8, 2024, at the above located workplace, a recordable injury occurred in the processing room. The employee sustained left hand inflammation pain, resulting in seven days away from work. The employer did not record the injury on an OSHA 300 Form, OSHA 301 Incident Report, or their equivalent form for calendar year 2024.
