At the warehouse, located at 6 Gum Street, in Fernandina Beach, FL, 32034: On or about October 30, 2024, the employer exposed employees to struck by and crushed by hazards, in that refresher training after a near-miss accident was not provided for powered industrial truck operator involved in an accident on October 7, 2024.
