29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. The employer is failing to protect employee(s) from fall hazards. Employee(s) were engaged in commercial roofing activities on or about October 28, 2024, at 7700 South 43rd Street, Bellevue, Nebraska. Employee(s) were exposed to fall hazards greater than six feet when roofing a commercial building without using any form of fall protection when working around unprotected roof openings, including skylights.
