29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a.) Jobsite, 300 Shadeland Avenue, East Pittsburgh, PA 15112: On or about October 17, 2024, an employee fell off a ladder, hit his head and was hospitalized. Five days later the employee passed away. The employer did not report within 24-hours a work-related incident resulting in an in-patient hospitalization.
