The WYMAN GORDON CO. was previously cited for violation of this Occupational Safety and Health Standard, or it's equivalent standard, 5(a)(1), which was contained in OSHA Inspection No. 107453268, Citation No. 01, Item No. 1(h)(1), issued on 11-19-91. Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: A. BEING STRUCK BY OVERHEAD LOAD AND OR OVERHEAD HOISTS AND ASSOCIATED PARTS WHEN OVERHEAD HOISTS WERE USED TO LIFT MATERIAL THAT WEIGHED MORE THAN THE RATED CAPACITY OF THE HOIST. OVERHEAD PENDANT CONTROL P & H HEVI-LIFT HOIST (W. G. #1165), LOCATED IN THE CONDUCTIVITY AREA OF THE HEAT TREAT DEPARTMENT WAS USED TO LIFT W.G. PART #15848 THAT EXCEEDED THE RATED CAPACITY OF THE HOIST. THE P & H HOIST (W.G. #1165) WAS RATED AT 2,000 LBS. AND THE W.G. PART #15848 WEIGHED 2,070 LBS. A FEASIBLE AND USEFUL METHOD TO CORRECT THIS HAZARD WOULD BE TO INSURE THAT EACH OPERATOR KNOWS THE WEIGHT OF THE MATERIAL THAT IS BEING HANDLED AND INSURE THAT NO MATERIAL WEIGHING IN EXCESS OF THE RATED LOAD OF THE HOIST IS LIFTED.
