29 CFR 1904.29(b)(1): The log of occupational injuries and illnesses, (OSHA Form No. 300), was not completed in the detail provided in the form and the instructions contained therein: Gerke Excavating, Inc - 15341 State Highway 131, Tomah. On or about October 16, 2024, the employer did not provide the injury event location (column E), provide a description to identify the object or substance that directly caused the injury (column F), and/or classify the case type (columns G-J) on the OSHA Form 300 or equivalent for calendar year 2024. (a) On or about September 10, 2024, an employee working in Rockport, TX sustained a cut to the top of their left thumb, case no. 8. The object or substance that directly injured the person was not described in column F. (b) On or about August 28, 2024, an employee working at an unidentified location had a callus on their left foot become infected after it was rubbed off, case no. 7. The injury event location was not identified in column E. (c) On or about August 28, 2024, an employee working at an unidentified location had a callus on their left foot become infected after it was rubbed off, case no. 7.
