29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: a) At 2733 Gunter Park Dr. West: On or about October 9, 2024 and at times prior, the employer exposed employees to struck-by hazards in that employees were allowed to operate a powered industrial truck without the employer ensuring that the operators had received the required evaluation at least once every three years as specified in the regulation. Price Rubber Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(l)(4)(iii), which was contained in OSHA inspection number 1483447 citation number 1 item number 1 and was affirmed as a final order on 9/21/2020 with respect to a workplace located at 2733 Gunter Park Dr. West.
