29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. a) Building 5 - Link @ W Squared, West Windsor NJ: Company worksite employees were exposed to fall hazards, in excess of 13 feet 3 inches & in excess 15 feet to the ground surface, when working/walking from a supported scaffold whose end frame openings were not supplied with guarding. Observed on or about 09/30/24. b) Building 5 - Link @ W Squared, West Windsor NJ: Company worksite employees were exposed to fall hazards, in excess of 11 feet 9 inches & in excess of 18 feet the ground surface, when working/walking from a supported scaffold side brackets's that were not provided with end stops. Observed on or about 09/30/24. c) Building 5 - Link @ W Squared, West Windsor NJ: A company worksite employee was exposed to fall hazards, in excess of 13 feet 3 inches to the ground surface, when working/walking from a supported scaffold whose walking/working surface was not supplied with mid rails. Observed on or about 09/30/24. d) Building 5 - Link @ W Squared, West Windsor NJ: Company worksite employees were exposed to fall hazards, in excess of 14 feet 6 inches to the ground surface, when working/walking from a supported scaffold's whose walking/working surface was not supplied with top rails. Observed on or about 09/30/24. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
