Text For Citation: 02 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by an elevator counterweight, crushed by falling into the space between the elevator shaft wall and the elevator car and an open elevator shaftway door. Exposure to these hazards could result in fractures and death. a) BOF Building, Furnace Aisle, Passenger Elevator - Employees were required to operate and ride the passenger elevator while sitting or standing on top of the elevator car itself, in the maintenance mode. Two feasible and acceptable methods to correct this hazard are to use the elevator as it was intended to be used (passengers ride inside elevator car and interlocks are not bypassed) or cease using the passenger elevator and use the stairwell to ascend and descend to the upper floor.
