29 CFR 1910.28(b)(3)(i): 29 CFR 1910.28(b)(3)(i): The employer did not ensure that each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following methods listed in 29 CFR 1910.28(b)(3)(i)(A)-(D): On or about 23 August 2024, and at times prior thereto, the employer instructed employee to create open holes for grain to flow into the silo, that exposed them to falls greater than four feet to the next lower level.
