Text For Citation: 02 Item/Group: 001 Hazard: LOCKOUT
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: On or about 4/14/86 the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: The the hazards of being caught in or crushed while working in and around 700 Ton Cincinnati milacron A02 #165 Plastic Injection molding machine, located in Plant #6, molding department during maintenance and production malfunction. Among other methods, one feasible and acceptable method to reduce these hazards is to establish and enforce an adquate lockout procedure which would include the following as referenced by ANSI Standard Z244.1: Lock out shall include but not be limited to the following procedures: 1) Lockout the main power disconnect for the plastic molding machine. 2) Establish a written lockout procedure including procedures to be followed and training for the affected employees. 3) Periodic evaluation and strict enforcement of all written provisions.
