29 CFR 1904.39(a)(1): The employer did not report the death of an employee resulting from a work-related incident within 8 hours of learning of the death: On or about August 14, 2024, at approximately 3:45PM an employee performing electrical work for Murphy Grain LLC, was fatally injured when contact live electricity. The employer failed to report the fatality to OSHA within eight (8) hours of being notified of the passing away of the employee. OSHA was not notified until August 15, 2024, at 6:40PM outside of the 8-hour reporting requirement.
