29 CFR 1904.41(a)(1)(i): 29 CFR 1904.41(a)(1)(i): The employer, with an establishment of 20-249 employees at any time during the previous calendar year, and in an industry listed in appendix A of 29 CFR 1904 Subpart E, did not electronically submit information from the OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA, or OSHA's designee, no later than the date listed in 29 CFR 1904.41(c) of the year after the calendar year covered by the form. a) 731 Avenue H Columbia, PA 17512 - On or about August 8, 2024, the employer, with 37 employees and a NAICS code of 327215, did not electronically submit information from the CY 2023 OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee by the deadline of March 2, 2024.
