29 CFR 1910.132(d)(1):The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): a) Closed Loop Plastic Recycling located at 1150 State Street, Fostoria, Ohio, 44830: On or about July 23, 2024, the employer failed to ensure that a hazard assessment identifying and selecting appropriate personal protective equipment was conducted prior to employees performing work activities such as, but not limited to, operating equipment and machinery on line 3. Employees operating line 3 without an employer assessment were exposed to bodily injuries from hazards such as flying particles, lacerations, and struck by hazards.
