1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation. a) On or about July 16, 2024, in the production area, the employer failed to lock out a Daniels Food Equipment DMX Quad Action 200 Mixer during setup. The equipment's interlock sensors were inoperable, did not prevent employees from operating the machine while in the zone of danger during the setup process in which the mixer was plugged in. Employees were, thereby, exposed to a caught in-between hazard.
