29 CFR 1904.29(a):Basic requirement. You must use OSHA 300, 300-A, and 301 forms, or equivalent forms, for recordable injuries and illnesses. The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report. a. On or about June 21, 2024, the employer did not ensure than an injury suffered by an employee that resulted on medical care beyond first aid, was recorded on the OSHA injury and illness forms. The employee suffered serious injuries when their hand was pulled into the in-going nip point between cylinders on the Champion Envelope Machine Model-R.A. Rotamatic Serial No. 4043 (Green Machine).
