Section 5(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushing hazards created by a structural collapse: (a) On or about June 7,2024 and at times prior to, at a construction site located at or near SW of I-94, exit 208, the employer did not ensure that employees were protected from struck-by and crushing hazards while setting the 80-foot-wide roof trusses on a commercial property without proper installation of temporary bracing, without a temporary restraint/bracing system designed by a Registered Design Professional Engineer, and without installing the permanent bracing required by the design engineer's plans. Employees working on, near, and underneath the truss systems were injured as a result of the structural collapse of the roof structure during truss setting operations. Abatement Note: Acceptable and feasible means to abate this hazard include following the manufacturer/design engineer building plans and referring to the Building Component Safety Information ("BCSI") manual for wood truss bracing requirements.
