29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a. At 1215 Imeson Park Blvd in Jacksonville Florida. On or about June 4, 2024, in the 12-foot deep trench, the employer exposed employees to cave-in, crushing, struck-by, and engulfment hazards from the trench sidewalls in that the employer had not provided a protective system to prevent cave-in.
