29 CFR 1910.178(l)(6): The certification did not include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation: (a) The Home Depot, 2445 Springfield Avenue Vauxhall, NJ 07088 On or about May 31, 2024 the employer failed to ensure that certifications include the identity of person(s) conducting forklift training or evaluation when an employee incidentally struck a co-worker while operating a forklift unsafely and causing a serious injury. Upon review of submitted documentation, name of the person who conducted the evaluation for the forklift operator was omitted.
