29 CFR 1904.39(a)(1):The employer did not report within 8-hours the death of an employee resulting from a work-related incident: a) Jobsite located at 140 N 50th St, Philadelphia, PA 19139: On or about April 25, 2024, an employee died as a result of blunt force trauma after working from a ladder to repair siding on the second story of a residential renovation and falling approximately 20 feet. The employer did not report the work-related fatality to OSHA within 8 hours.
