Text For Citation: 02 Item/Group: 001 Hazard:
29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of hazardous energy when employees were engaged in activities such as servicing and/ or maintenance of equipment: Primer Room: On or about April 30, 2024, the employer exposed an employee to amputation hazards in that the employer failed to ensure specific lockout tagout procedures were utilized while cleaning the GFG coil rollers resulting in severe hand injuries. Specrail was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 1477516, citation number 1, item number 1 and was affirmed as a final order on December 30, 2020, with respect to a workplace located at 129 Leeder Hill Road Hamden, CT 06517.
