29 CFR 1910.23(b)(10): The employer did not ensure that any ladder with structural or other defects was immediately tagged "Dangerous: Do Not Use" or with similar language in accordance with 1910.145 and removed from service until repaired in accordance with 1910.22(d), or replaced: On or about April 24, 2024, the following fixed ladders were not immediately tagged "Dangerous: Do Not Use" or with similar language and were available for employee use: a. In the south boot pit, a fixed metal ladder that bent upward at the lower rungs. b. In the north boot pit, a fixed metal ladder was missing ladder rungs.
