29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). (a)Stone Hill Excavation, LLC at 351 South Douglas Street, Salem, SD 57058: On or about and at times prior to April 22, 2024, the employer did not ensure employees conducting sewer pipe connection and installation within an unprotected excavation greater than five feet in depth were adequately protected from trench wall cave-in. This condition exposed employees to struck-by and cave-in hazards associated with a trench wall collapse.
