29 CFR 1904.29(a): The employer did not use an OSHA 300, 301 and 300A Form or equivalent: a) Worksite - On or about April 17, 2024, an employee handled a piece of aluminum metal flashing/drip edge on an Alum-A-Pole pump jack scaffold that contacted an uninsulated power line with voltage of 13.2kV phase to phase and 7.62kV phase to ground exposing the employee to electrocution hazards. The employee was hospitalized and missed days away from work. The employer did not use an OSHA 300 Log or OSHA 301 or equivalent to record the injury.
