29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: a) In the worksite, 160 Gamma Dr. Pittsburgh, on or about April 12, 2024 - The employer did not provide Appendix D of the standard to production employees from Plant 1 and Plant 2 who voluntarily used 1/2 face air purifying respirators, and full face respirators. Employees were exposed to hazardous chemicals such as but not limited to : Sil-Co-Sil which contained crystalline silica 95-99.9%; Sodium Fluorosilicate 98%; Silicic acid 75%; D.E.H. 570 Epoxy Curing Agent; Milled zircon which contained zirconium silicate and quartz silica; Ref-Bond MP2 which contained amorphous silica; Magnesium oxide and Boric acid 99.9% during production tasks.
