29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about April 9, 2024, located at 738 East Monroe Avenue, Hartford, WI 53027, the employer failed to ensure employees installing shingles on the second level of the residential condo were protected from falls by using a guardrail system, safety net system, or personal fall arrest system, exposing employees to a fall hazard of approximately 18 feet. (b) On or about April 9, 2024, located at 738 East Monroe Avenue, Hartford, WI 53027, the employer failed to ensure employees installing underlayment and shingles on the first level of the residential condo were protected from falls by using a guardrail system, safety net system, or personal fall arrest system, exposing the employees to a fall hazard of approximately nine (9) feet. Alexander Hernandez DBA A&A Roofing & Siding LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1687713, citation number 1, item number 1a, and was affirmed as a final order on 11-08-2023, with respect to a workplace located at 6550 N Meade St, Appleton, WI 54913. Alexander Hernandez DBA A&A Roofing & Siding LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1593743, citation number 1, item number 1 and was affirmed as a final order on 09-28-2022, with respect to a workplace located at 1198 Olson Ave, Oshkosh, WI 54901.
