29 CFR 1910.28(b)(6)(i): The employer did not ensure that each employee less than 4 feet (1.2 m) above dangerous equipment is protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system, unless the equipment is covered or guarded to eliminate the hazard. At 18594 Hwy KK in Boss, MO 65440 (Refinery) - On or about 27 March 2024, employees of The Buick Resource Recycling Facility (BRRF), DBA Doe Run, were exposed to fall hazards on the kettle deck posed by open kettles without any means of fall protection.
