29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:(a) Location: Facility production area On or about March 21, 2024, employees were exposed to hazards associated to the unintentional release of energy due to the lack of a functioning Lock Out/Tag Out program with machine specific procedures. Employees performed cleaning of equipment without affixing lockout tagout equipment to prevent the unintentional energizing of the equipment while employees were working on the following equipment: 1. While employees clean the Hallmark Tilt Bowl Mixer the employee cleaning the mixer did not affix an energy control device (lock) to the main disconnect switch located on the wall behind the mixer while cleaning the equipment. 2. While employees clean the Champion Tilt Bowl Mixer the employee cleaning the mixer did not affix an energy control device (lock) to the main disconnect switch located on the wall behind the mixer while cleaning the equipment. The Chinese Spaghetti Factory was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(1), which was contained in OSHA inspection number1656265, citation number1, item number 1 and was affirmed as a final order on September 19, 2023, with respect to a workplace located at 83 Newmarket Square.
