29 CFR 1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Plant 1- On or about January 23, 2024, the employer did not ensure that all the steps outlined in 29 CFR 1910.147(d), required for the control of the hazardous energy sources, were performed. Employees did not de-energize and lockout mechanical hazardous energy sources of all equipment, such as the Line 1 Weber 804, prior to conducting cleaning and sanitation activities. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)], and verification of isolation [1910.147(d)(6)] are not implemented to protect employees from mechanical hazards while employees are conducting cleaning and sanitation activities on the Line 1 Weber 804. Employees were exposed to mechanical hazards. b) Plant 2- On or about January 23, 2024, the employer did not ensure that all the steps outlined in 29 CFR 1910.147(d), required for the control of the hazardous energy sources, were performed. Employees did not de-energize and lockout mechanical hazardous energy sources of equipment, such as the Scale Infeed Conveyor, prior to conducting cleaning and sanitation activities. As a result, the remaining applicable energy control elements, involving the shutdown of the equipment [1910.147(d)(2)], machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from mechanical hazards while employees were conducting inspection cleaning and sanitation activities on the Scale Infeed Conveyor. Employees were exposed to mechanical hazards. No abatement certification or documentation is required for this item.
