29 CFR 1910.178(a)(5):The Powered Industrial Truck (PIT) was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: (a)Lloyd Companies dba Engineered Truss Systems, LLC. at 1400 East Benson Road, Sioux Falls, SD 57104: On or about and at times prior to January 17, 2024, the employer did not ensure that forklifts were marked to identify changes in weight and performance while fork extensions not provided by the manufacturer were installed. This condition exposed employees to crush-by hazards associated with unexpected forklift performance.
