29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by in-going nip points, rotating parts, and/or flying chips and sparks: (a) Extrusion area at Machine 4 Winder: On or about December 19, 2023 and at times prior, employee(s) were exposed to caught in or between hazard(s) in that the employer did not ensure Machine 4 Winder was guarded in such a manner as to prevent employees from reaching into the point of operation. Crayex Corporation was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 1532775, citation number 1, item number 1, and was affirmed as a final order on 09-22-2021, with respect to a workplace located at 1747 Commerce Drive, Piqua, OH 45356.
