29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) At the Shape Chipper: The employer fails to ensure effective measures to prevent caught-in and struck-by hazards as demonstrated on or about December 18, 2023, where employees engage in maintenance activities between the left and right heads, such as, but not limited to, clearing a jam. To abate these hazards, the employer must ensure that written hazardous energy control procedures are developed, documented and utilized at the shape chipper.
