29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a) For the Schnell rebar shear and bender, on or about 12/05/2023, the employer exposed employees to amputation hazards, in that, employees were exposed to the rotating rebar bender component of the machine during its normal operation and during rebar feed reversal followed by fault codes output by the machine. Cast-Crete USA, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1416547, citation number 1, item number 2 and was affirmed as a final order on 09/30/2019, with respect to a workplace located at 6324 County Road 579, Seffner, FL 33584.
