29 CFR 1904.29(a): A log of all Work-Related Injury and Illnesses (OSHA Form 300), and/or the Summary of Work-Related Injury and Illnesses, (OSHA Form 300-A), and/or the Injury and Illness Report (OSHA Form 301) or equivalent forms were not kept by the establishment: a) Hiossen Inc. - 401 Plymouth Rd., Plymouth Meeting, PA - On or about December 11, 2023, a Dental Implant Sales Rep suffered a recordable injury as a result of a contaminated needlestick. The employer did not generate and maintain the required OSHA forms 301, 300, and 300A for the 2023 calendar year. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
