29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a)T & J Industries, Inc dba Metal Craft Industries at 1014 South Hillside Drive, Sioux Falls, SD 57110: On or about and at times prior to December 6, 2023, the employer did not ensure the Haas VF-3 CNC Milling Machine in the machine area of the facility was properly guarded. This condition exposed employees to caught-in and amputation hazards associated with bypassing the machine's interlock system allowing the machine to operate when the doors are opened exposing the area of unguarded mechanical action. This condition exposed employees to caught-in and amputation hazards. b)T & J Industries, Inc dba Metal Craft Industries at 1014 South Hillside Drive, Sioux Falls, SD 57110: On or about and at times prior to December 6, 2023, the employer did not ensure the Haas VF-4SS, CNC Milling Machine in the machine area of the facility was properly guarded. This condition exposed employees to caught-in and amputation hazards associated with removing the machine's interlock system allowing the machine to operate when the doors are opened exposing the area of unguarded mechanical action. c)T & J Industries, Inc dba Metal Craft Industries at 1014 South Hillside Drive, Sioux Falls, SD 57110: On or about and at times prior to December 6, 2023, the employer did not ensure, the OKUMA Lathe 762S-BB, Serial 1855, in the machine area of the facility, interlock was functioning properly and the metal rod feeder was properly guarded. This condition exposed employees to caught-in, stuck-by, caught-in and amputation hazards associated with bypassing the machine's interlock system allowing the machine to operate when the doors are opened, and operating the metal rod feeder without a guard, exposing the area of unguarded mechanical action. d)T & J Industries, Inc dba Metal Craft Industries at 1014 South Hillside Drive, Sioux Falls, SD 57110: On or about and at times prior to December 6, 2023, the employer did not ensure the OKUMA Lathe 762S-BB, Serial 1310, in the machine area of the facility did not ensure the interlock was functioning properly and the metal rod feeder was properly guarded. This condition exposed employees to caught-in, stuck-by, caught-in and amputation hazards associated with bypassing the machine's interlock system allowing the machine to operate when the doors are opened, and operating the metal rod feeder without a guard, exposing the area of unguarded mechanical action. e)T & J Industries, Inc dba Metal Craft Industries at 1014 South Hillside Drive, Sioux Falls, SD 57110: On or about and at times prior to December 6, 2023, the employer did not ensure the interlock was functioning properly or that the turret and tailstock of the Okuma Captain L370 Lathe, Serial 106866, in the machine area of the facility was properly guarded. This condition exposed employees to caught-in, amputation electrocution hazards associated with bypassing the machine's interlock system allowing the machine to operate when the doors are opened, and operating the machine with the turret and tail stock without a guard, exposing the area of unguarded mechanical action. f)T & J Industries, Inc dba Metal Craft Industries at 1014 South Hillside Drive, Sioux Falls, SD 57110: On or about and at times prior to December 6, 2023, the employer did not ensure the discharge end of the Hennig KMO-0320-00 Chip Conveyor, Serial 9760 in the machine area of the facility was properly guarded with a feeder end flip lid. This condition exposed employees to caught-in and amputation hazards associated with the exposing area of unguarded mechanical action. g)T & J Industries, Inc dba Metal Craft Industries at 1014 South Hillside Drive, Sioux Falls, SD 57110: On or about and at times prior to December 6, 2023, the employer did not ensure the discharge end of the Hennig KMM-1360-01 Chip Conveyor, Serial 100874 in the machine area of the facility was properly guarded with a feeder end flip lid. This condition exposed employees to caught-in and amputation hazards associated with the exposing area of unguarded mechanical action. h)T & J Industries, Inc dba Metal Craft Industries at 1014 South Hillside Drive, Sioux Falls, SD 57110: On or about and at times prior to December 6, 2023, the employer did not ensure that a guard panel on the Amada, FANUC AF4000E, a laser cutter, Serial L02440434, in the CNC laser cutting area of the facility was properly guarded. This condition exposed employees to caught-in and amputation and electrical hazards associated with the exposing area of unguarded mechanical action. i) T & J Industries, Inc dba Metal Craft Industries at 1014 South Hillside Drive, Sioux Falls, SD 57110: On or about and at times prior to December 6, 2023, the employer did not ensure that a chuck guard on the Tool Shop GLT120300051, 13-inch drill press, in the machine area of the facility was properly guarded. This condition exposed employees to caught-in and amputation hazards associated with the exposing area of unguarded mechanical action.
