29 CFR 1904.39(a)(1):Basic Requirement. Within eight (8) hours after the death of any employee as a result of a work-related incident, you must report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor. a. On or about December 4, 2023, located at the jobsite 4547 West Northbank Road, Buckeye Lake, Ohio, the employer did not notify OSHA of a fatality within 8 hours of being notified of the event.
