29 CFR 1904.29(b)(1): The log of occupational injuries and illnesses, (OSHA Form No. 300), was not completed in the detail provided in the form and the instructions contained therein: Eau Claire Insulation, LLC - 1125 Starr Avenue, Eau Claire, WI. On or about November 29, 2023, the employer did not provide the required information in column F to identify what object/substance directly injured each employee and did not place a mark in column G, H, I, or J to classify the case as a death, days away from work, job transfer/restriction, or other recordable case on OSHA Form 300 for calendar year 2023. (a) On or about November 22, 2023, while working at the Ladysmith School, an employee received a hernia. (b) On or about October 3, 2023, while working at the ECI shop, an employee received a head/scalp abrasion, and the case type was not classified. (c) On or about September 27, 2023, while working at the ECI parking lot, an employee received a right knee laceration, and the case type was not classified.
