Sebastian, West Helena, AR 72390: On or about November 28, 2023, an OSHA Form 301 or equivalent for the following work-related injuries or illnesses was not filled out for the calendar year 2023: On November 28, 2023, an employee sustained 1st and 2nd degree burns to the face, both hands and forearms from a propane fire, resulting in days away from work. The equivalent form being used was not complete in that fields including, but not limited to the following were blank: a) Contact Name/Address b) Date of birth c) Gender of employee d) Name of physician or other health care professional e) If treatment was given away from the worksite, where was it given? f) All Equipment, materials, or chemicals employee was using when accident or illness exposure occurred
